
The HOPE (Healthy Opportunities through Prevention and Empowerment) Service is a part of the South West Hospital and Health Service prevention work, building on and expanding the previous Healthy Communities approach across the region. HOPE focuses on prevention and early intervention, working alongside communities and partners to support health and wellbeing across the life span.
As part of this work, we are seeking input to better understand the health and wellbeing needs of our communities, from young children through to older adults. Your input will help shape future programs and services to better support strong and healthy communities.
