My Learning Passport is designed to support people with intellectual or learning disability to get ready for the NDIS. It is based on the successful face to face Plan ahead workshops, delivered around Queensland in 2015, for QDN’s Ready to go project, an NDIS Participant Readiness activity funded by the Department of Communities, Child Safety and Disability Services.
The series of 8 short videos features presentations by two peer facilitators Sam Matheson and Kane Wojicicki, graphic animation and activities, fun and games along the way. Participants will learn about the Pathways to the NDIS, choice and control, the life domains, the planning conversation, what is reasonable and necessary and managing their plan.
Participants can learn at their own pace, on their own, or with the help of a family member, friend or support person.
My Learning Passport resources include a participant workbook and supporter’s guide. Audio versions and DVDs are also available.
